Service Plan Updates

  • Pregnancy comes with unexpected changes, so your care plan needs to be flexible enough to change with you.

  • The Action: We will modify your active Service Plan based on your current physical health and daily life.

  • What It Means: If a doctor diagnoses you with a new high-risk flag (like gestational diabetes) or if you experience a sudden emergency (like a utility shutoff notice), we will instantly add new goals, request medical monitoring equipment, or contact emergency funds to help you.

Next Steps & Referrals
(Your Action Plan)

  • We wrap up the visit by making sure you have a clear, stress-free roadmap until our next monthly check-in.

  • The Action: We will schedule your next round of support services and hand off any required phone numbers or applications.

  • What It Means: We will coordinate upcoming high-risk specialist referrals, set up transport rides for your next OB/GYN visit, or connect you with local parenting classes and doula support networks

  • You can rapidly tell us how you are feeling, share news from your recent doctor visits, or let us know if your health has changed.

We will quickly verify if your food benefits (WIC/SNAP), housing applications, or baby supply requests are moving along smoothly.

If life throws you a curveball, you can immediately request an update to your service plan—such as adding a new high-risk referral, ordering home medical equipment, or getting help with an emergency bill.

If you need additional services, this appointment slot can also be used to make a new service request.

The Process

Apply & Review:

You fill out our quick eligibility/referral form online. We will carefully review your information within 24 hours.

Ongoing Follow-Up Care:

  • We will meet for routine check-ins either in person, by phone, or via telehealth as often as you need to accomplish your goals, update your service plan, and address your needs.

Welcome Call:

Once you are determined eligible, we will call you directly to discuss the referral and find a convenient time to schedule your Initial Comprehensive Visit.

Successful Case Closing:

  • Once your baby arrives safely

  • your goals are achieved

  • all your medical or financial needs are fully met, we will officially close your care file.

Initial Comprehensive Visit:

A 60-minute visit online via telehealth or in-person. Together, we will complete your consent, look over your health history, and complete a Family Needs Assessment to look at the big picture of your daily life
