What we do: We will sit down and review a standard Service Plan Consent Form together.

What it means: This form explains that our program is 100% free and voluntary. Joining will never change or hurt your regular Medicaid benefits, and you can stop working with us at any time if you choose.

The goal: We get your official permission (a quick signature) to start helping you and sharing information safely with your doctors.
Family & Medical History
(Your Health Journey)

What we do: We will chat about your health history, any past pregnancies, and how you are physically feeling right now.

What it means: You can share if you’ve had complications before (like high blood pressure or early labor), any health conditions you manage daily (like diabetes), or medications you take.

The goal: We use this to see if you need special doctors, extra ultrasounds, or specific medical supplies covered by your insurance.
Needs Assessment (Your Everyday Life)

What we do: We will complete a Family Needs Assessment to look at the big picture of your daily life.

What it means: We will safely talk about things like housing stability, having enough healthy food, finding rides to appointments, paying utility bills, or managing stress, anxiety, and safety at home.

The goal: We find out exactly where life is feeling heavy right now so we can instantly connect you to community resources like food programs (WIC/SNAP), housing aid, free baby gear, or counseling support.
The Process

Apply & Review:
You fill out our quick eligibility/referral form online. We will carefully review your information within 24 hours.

Ongoing Follow-Up Care:

We will meet for routine check-ins either in person, by phone, or via telehealth as often as you need to accomplish your goals, update your service plan, and address your needs.
2.
Welcome Call:
Once you are determined eligible, we will call you directly to discuss the referral and find a convenient time to schedule your Initial Comprehensive Visit.

Successful Case Closing:

all your medical or financial needs are fully met, we will officially close your care file.

Initial Comprehensive Visit:
A 60-minute visit online via telehealth or in-person. Together, we will complete your consent, look over your health history, and complete a Family Needs Assessment to look at the big picture of your daily life
